What happened to 'insert attachment' button?

I was about to post a new topic, but couldn't attach images to it. I've tried it with Waterfox and Firefox, same result with both.

I wonder is it on my end, or has something changed, like forum software update or something like that?

Yeah, that's visible for me as well, but 'insert attachment' - well not actually a button - doesn't show up. What I mean is shown in the pic from another SMF forum:

Maybe a dumb question, but in case someone else wonders also, how did you attach images to your recent post?

Is it just by referencing to attachments like [attachment=1] in the post?

Still a problem to insert an image inline. Also for existing posts with inline images, like this one, the post is messed up with text references to the inline images, which don't appear inline anymore:
