A couple of pictures of your knurling tool in situe, showing alignment and one with a bit of stock between knurls before knurling.

As for the pins this is my first thoughts.
The end of the highlighted shaft will be threaded to secure them in place which will be set by a number of spacers I can put either side dependent on the stock I am turning. The keyway will allow me to fully tighten it without it turning.
Inside will be a sprung brass bar with a screw to set it against the stock.

I assume that the spring is just there to stop the insert falling out?

How much adjustment will you have before needing to swap spacers?
The spring will hold the pins back so the stock can be fed through without them getting in the way and I am thinking of somewhere in the region of 20-25mm before I have to swap the spacers to get the next 20-25mm. quickly mocked up the pin with freecad still need to go through the details in my head yet however.

I don't see how that could make any difference.  I don't really see why the tool would have been upside down either - I think even when used in a rear tool post it would be fine to run the lathe forward so the tool could still be the right way up.

You have made me wonder though if there is any possibility of misalignment between the knurls.  If they were able to move sideways or the points of the diamond pattern were out of line then that might cause a problem.

I assume you are using diamond knurls?  You can do a diamond pattern with straight knurls at 45 degrees.

The knurls never bit down hard enough, what is your material and has it work hardened ?

Another point, were the knurls clean of swarf, metal particles.

The Steel is EN1 mild bright I cleaned everything before hand and used lots of oil , I am wondering it moving tool to the rear made the difference because I had to take everything apart and put it back together the other way round otherwise the adjuster would have been on the bottom and impossible to adjust.

The hex doesn't look thick enough to properly support the pins. I'd probably go for twice the diameter. You could always weld bushes or screw them in.

I did think about making it split but decided against that so it will be a case of feeding the bar through as best as I can. I must admit rigidity is one of my concerns which I hopefully will be able to cope with once I have welded this hexagon up, Maybe with the use of some extra round plates front and back.
